USMLE Fees 2027: Step 1 and Step 2 CK Remain at $695 as New Eligibility Period Opens

The United States Medical Licensing Examination (USMLE) will retain its application fees for Step 1 and Step 2 Clinical Knowledge (CK) at US$695 each for 2027. The new fee schedule applies to three-month eligibility periods ending in 2027, with the first period running from November 1, 2026, to January 31, 2027. Indian medical graduates should also account for applicable GST and international testing surcharges.

USMLE 2027 Application Fees

The Federation of State Medical Boards (FSMB) has published the application fees for the 2026 and 2027 examination cycles. The Step 1 and Step 2 CK fees remain unchanged at US$695 each.

The fees above exclude applicable regional charges and taxes. Candidates should verify the final payable amount through the relevant registration portal before submitting payment.

When Does the USMLE 2027 Fee Schedule Apply?

The 2027 fee schedule applies to three-month eligibility periods ending in 2027. The first such period begins on November 1, 2026, and ends on January 31, 2027.

Candidates should select their preferred eligibility period while submitting the examination application. The available testing dates depend on the selected period and appointment availability at Prometric centres.

USMLE Fees for Indian Medical Graduates

Indian medical graduates applying for Step 1 or Step 2 CK should consider additional costs beyond the base examination fee.

The regional fee applies to examinations taken outside the United States and Canada. For applicants selecting India as their testing region, the USMLE programme collects 18% GST on Step 1 and Step 2 CK under the applicable Indian tax rules.

The final amount can therefore vary according to the examination, testing location and applicable taxes. Candidates should check the official fee page for the latest payment details.

How to Apply for USMLE 2027

Medical students and graduates must apply through the registration organisation designated for their medical school.

Applicants from medical schools outside the United States should use the FSMB registration route. They must also verify that their medical school meets the applicable ECFMG eligibility requirements.

Before applying, candidates should review the official USMLE eligibility criteria and the 2027 Bulletin of Information. Once their application is approved, they can use their scheduling permit to arrange an examination appointment.
